Local Referral and Resource Directories
The purpose of the local referral and resource directory developed for our recruitment sites (Hertford and Edgecombe-Nash counties), is to enhance communication and referrals among CHANGE-affiliated agencies in the counties. The directories include local resources related to the intervention’s goals of improving diet, increasing physical activity, stopping tobacco use, and increasing medication adherence. Each resource listing includes detailed information on when and where resources are available and an individual to contact. In addition to the Community Health Workers keeping a paper copy of the resource directory, the directory is also included in the participant handbook.
